IDYOCF 將就閣下剔選的興趣項目向會員推廣相關活動，並會為會員感興趣的項目組織活動。IDYOCF will promote relevant activities to members based on the interests ticked, and will organise activities for items members are interested in.

C、聲明、同意及條款C. Declaration, Consent & Terms
  1. 本人確認上述資料真確無誤，並自願申請成為 IDYOCF 會員。I confirm the information above is true and accurate, and voluntarily apply for IDYOCF membership.
  2. 個人資料（私隱）條例：本人同意 IDYOCF 依香港法例第486章《個人資料（私隱）條例》（PDPO）收集、使用、保留及在必要時轉交上述個人資料，僅作會員管理、活動通知、服務安排及相關慈善用途。PDPO: I consent to IDYOCF collecting, using, retaining and, where necessary, transferring my personal data under the Personal Data (Privacy) Ordinance, Cap. 486, solely for membership administration, activity notifications, service arrangements and related charitable purposes.
  3. 本人有權根據 PDPO 查閱、更正或要求刪除所提供之個人資料；查詢請以書面聯絡 IDYOCF。I have the right under the PDPO to access, correct or request deletion of my personal data; enquiries should be made to IDYOCF in writing.
  4. 本人同意 IDYOCF 使用本人資料（含姓名、地址、電話、電郵及活動紀錄）向本人發送會務及活動資訊；本人可隨時要求停止。I agree IDYOCF may use my data (name, address, tel, email, activity records) to send me updates; I may opt out at any time.
  5. 本人知悉並遵守 IDYOCF 章程及會員守則；IDYOCF 保留接納、拒絕或取消會籍的權利。I undertake to abide by IDYOCF's constitution and code of conduct; IDYOCF reserves the right to accept, reject or revoke membership.
  6. 活動免責：參與活動存在一定風險，參加者（或家長／監護人）須自行評估並自負風險；IDYOCF 對活動期間之意外、傷病或財物損失概不負責。Activity Disclaimer: Participation involves inherent risks; participants (or parents/guardians) assess and assume such risks; IDYOCF accepts no liability for injury, illness or loss.
  7. 未滿18歲的申請人須獲家長／監護人於下方簽署同意。Applicants under 18 must obtain parent/guardian signature below.
